Immunization Requirements
for Enrolled Students

 

The State of Connecticut, Department of Public Health has certain immunization requirements for Enrolled Students in Connecticut Schools.

Required Physical Form

State of Connecticut Department of Education HEALTH ASSESSMENT RECORD (physical form - click link - pdf format)
Part 1 to be completed by parent, Part II to be completed by physician.  Form must be filled out and returned to the school nurse for entry to Kindergarten and during Grade 6 and Grade 9 (for entry to Grade 7 and Grade 10.)


 

Summer Health Services

All Middletown Students can seek summer services at the School Based Health Centers this summer!

The School Based Health Centers at Wilson, Macdonough, Keigwin and Middletown High School will be open starting on Monday, August 18, 2014, with appointments available to SBHC enrolled students in all locations for physicals, sports physicals and vaccinations.

Please note that students from other Middletown schools who don’t currently have a School Based Health Center can also seek services at Middletown High and Vinal Technical High School located at 60 Daniels Street, Middletown.
