The School-Based Health Center
offers three different programs as described below. 

The table indicates which program is available at an individual school.

All students are required to have a physical exam prior to entering PreKindergarten, Kindergarten, Grade 7 and Grade 10.  This exam and necessary vaccines and associated paperwork may be conducted at the school based health centers.  Click here for more information.


The Community Health Center, Inc. is pleased to offer school-based health services in your child’s school during the school day. Licensed healthcare providers are available to provide the following health care services:

     Medical

  • Expanded Medical (treatment for illnesses or injuries, physicals, vaccines, prescriptions)

     Behavioral Health

  • Behavioral Health (individual, group, family therapy)

     Mobile Dental

  • Dental Hygiene Services (exams, cleanings, fluoride treatments, sealants)
  • Dental restorative services (such as fillings, may also be provided)

School-based health services work in conjunction with the care provided by your child’s pediatrician and/or dentist and are not intended to replace regular care by your child’s primary health-care provider.
