Athletics

Middletown School Nurses recognize the value of participating in school-sponsored athletics programs and work to assist students and their families in attaining the required forms and permissions. However, the coaches in each sport run the athletic programs. They are responsible for selecting the team, coordinating practices and enforcing all procedures, including the physical/medical clearance policies.

Students wishing to participate in athletics (including practice) must submit documentation of a current physical examination complete with parent and health care provider signatures to the nurse for review and collaboration with coaches before a student is permitted to participate in any athletics programs.
