I9 Employment Eligibility Verification (Employees must provide two current forms of ID from the list)

Personal/Emergency Contact Sheet
Authorization for Release of Information for DCF CPS Search

Public Act 16-67

Direct Deposit (Employee should attach a voided check or savings deposit slip to this completed form)

FBI Privacy Act Statement

Paraprofessional Union Member Application

Hepatitis B Declination Form (to be completed if you do not want to receive the vaccination series)

Paraprofessional Manual (Please sign and return the last page to HR)

Online Employee Training

You must provide a copy of unofficial transcripts confirming that you have an Associates Degree, at least 60 credits or proof that you have taken and passed the ParaPro.

Once completed with all pre-employment paperwork and training's, please contact

You must be fingerprinted. Below is the link to the Fingerprint Procedure. Fingerprint cards are returned to the BOE HR Department.

Fingerprint Procedure

You must complete online employee training. Please go to the main HR webpage, click on Annual Employee Training. Enter the username and password provided to you and complete the entire page.
