The district of Middletown Public Schools has partnered with Jackson Labs, who will be visiting each of our schools on a predetermined day and time each week. If you consent to your student to be screened for COVID each week, a school staff member will bring them to a testing room within the school building. The Jackson Laboratory lab technicians will assist students with self-swabbing or complete swabbing for students who are unable to self-swab. Individual results will be provided to the students’ parent/guardian, school, and Department of Public Health (DPH) within 24-48 hours after testing.

How to Register: (The registration steps are the same whether you are a student or employee)

  1. Visit this website: https://uswemd.com/signup/middletownps This site is also the portal where you can view screening results.
  2. Register: Each user will need to be registered individually and will need a unique email address to register in the system.
    If you use GMail or Outlook for your email provider and you do not want to create new email addresses for your children, you can register using a task-specific email address; when registering, simply do the following to your current email address:
    Add a plus sign (+) and any word before the @ sign in your current email address; for example:
    Current Email address: [email protected]
    Task-Specific Email Address: JaneDoe+John@gmail.com or JaneDoe+Sally@gmail.com
    Messages sent to task-specific email addresses will be sent to your current email address
